How to Submit SFRF Project Performance/KPI Data to NCPRO
Step 1. Access Survey
You will receive an email from the domain @app.smartsheet.com containing a Data Collection Request link. Open the email and click the blue “Open Request” button. To ensure this email does not go to your spam or junk folder, you can add @app.smartsheet.com to a list of trusted email domains in Outlook.
Note: Data Collection Request emails will be automatically sent weekly until all KPI data that is due is submitted. Each email will contain all that is currently due. You only need to respond to the latest request. Ignore or delete any that are more than one week old.
Step 2. Enter KPI Values and Submit
- Before entering any data, be sure to check the Project Name, KPI Information, and Reporting Period for accuracy.
- For each KPI, add the cumulative data value (up to and including the specified reporting period) in the “KPI Value” box using numbers only (no text or special characters). The "Comment" box is optional if you have any additional information to provide about the data.
- If data can be disaggregated (as specified in the KPI Information section), put the aggregate amount in the “KPI Value” box, and upload an excel file with disaggregated data by clicking “Upload Files” at the bottom of the page.
- If there are multiple KPI values due at this time, there will be multiple screens to enter data, so click “Next” to move to the next screen.
- When all values are entered, click "Submit Update."
Note: If you are unsure about any of your KPI values, you can enter only the values you are sure about then click "Submit Update." Any missing values will be included in the next weekly automated Data Collection Request email link from NCPRO.

How to Add Smartsheet as a Trusted Email Domain in Outlook
The following steps provide instructions for how to add Smartsheet as a trusted email domain to ensure your Data Collection Request link is sent directly to your inbox.
- Under the Home tab in Outlook, select the "Junk" icon. A drop-down list will appear.
- From the drop-down list, select "Junk Email Options". A separate window will open for these options.
- Navigate to the tab at the top of this window labeled "Safe Senders".
- After you select this tab, select the "Add..." button. Another separate window, labeled "Add address or domain", will open with a text box.
- In the textbox in this window, enter the domain @app.smartsheet.com and then select "OK". This will take you back to the previous "Junk Email Options" window.
- Select "Apply".
